If you need a new tree to replace the dead one on the city property,
you need to apply for "Adopt-a-Tree program".

I had called the person in charged of the project many time to only
get the answering machine.

I surfed the city website and finally found the application forms.
It was not obvious to find. I figured that the Bellevue web site could
give some information about it and make the forms more easily available.

See below all forms required.

Can something be organized to make those forms available to the people
that don't have computer and could not get in touch the person in charge
at the City? For example, Copies of the forms could be available at the
next neighborhood meeting or put in a Bellevue public location.

P.S. I got the Bellevue map and got started on the trees survey.
